This is the kind of stuff that keeps me up at night, along with the worry that its not keeping others up. Just because you aren't building a Therac-25, doesn't mean your mistakes cannot have material impact on people's lives. As a software industry we have rushed so fast to delivery that QA is now done by customers. And that won't change until there is financial impact, which only comes after real pain and suffer years after the original developers are gone.
