Two relevant bits:

"If you go to the settings for FaceTime, there is an option to toggle Sensitive Content Warning on or off. It is off by default, indicating that it may not actually come to everyone’s phones, or at least that you’ll have an option to turn it off."

And:

"Images are analysed locally on the phone, with nothing transmitted to Apple’s servers."

unless of course they add an update to send the sensitive content to remote servers either by their will or forced by some government.

They have proven they can detect, now it's a matter of getting them to report.

- saudi arabia can try to force apple to report any such content where parties are of the same gender. (death penalty)

- china can try to to force apple to report calls where narcotics are detected within their borders with exact location (death penalty).
