I have been seeing it change drastically over the years, sometimes looking bloated, sometimes scarred and now in his interview with the NY Times, it looks like something is eating away the flesh on the side of his face.

Plenty of rumors out there. One is that he has temporal wasting, also called facial lipoatrophy. Can be associated with HIV and medications used to treat HIV.
