Sadly, the ship's sinking - https://en.wikipedia.org/wiki/HMHS_Britannic#Last_voyage_and... - sounds like To Do list for demonstrating incompetence. "Watertight" doors that didn't actually work, lower portholes left wide open as the sea outside rose and started pouring in, lifeboats lowered right into the path of the meat grinders (exposed & churning propellers), and more.
