Sometimes not even the outsourcing company's "fault" as humans / organizations love to think "oh I don't have to think about that now that I outsourced it" and folks who do worry about the detail then leave (they can't use their skills) and now you got folks in charge who don't even understand what they're agreeing to with their outsourcing company. Other times, it is the outsourcing groups fault ...
I'd wager it's likely that TCS staff who worked at Jaguar were ordered to do their best work.

Simple fix - as an entry fee for the emergency loan program, require that the corporation's C-suite give HM Treasury all their bonuses, stock options, & such from the past (say) 5 years. "Just aligning our incentives, chaps. His Maj doesn't hand out gongs for stuffing up."
