Show HN: Add legally binding signature to your Google Form

While launching products, I needed a lead form on our website. Google Forms had everything I wanted: easy setup, Google Sheets integration. But embedded on our site it looked like a patchwork. So I restyled it with CSS to match our branding. I launched that CSS hack as an addon and thousands of customers started using it. One customer segment, doctors, liked our addon but were worried about HIPAA. So we studied HIPAA regulations and built a HIPAA compliance product with e-signatures, PHI field masking and intake forms. But, only the signature feature took off. So, we renamed it as Formesign and pulled the other features back in around it and made it into a DocuSign alternative that lives inside Google Forms, syncs responses to Google Sheets and saves the signed PDF in Google Drive.
