Fecal transplant. The pre biotics are good, but it's changing the biota of the lower gut which I think does the magic.

The goal of course is to custom tailor probiotics in order to not need the fecal transplant to modulate. GLP-1s will get the weight off, this is what is needed to keep it off. Might not even need the GLP-1s eventually, just update the gut biome with the prescribed metabolic profile.

Pro and pre biotics have to transit the upper gut and stomach biome to get to lower gut to do their goodness. I consume both, and I am sure I feel better for it but from people I know with crohns and IBS the word is, fecal transplant gets you to a better place faster.
In my unscientific view based on a sample size of 1, resistant starch, which really just means starch like cooked spuds, pasta or rice which is allowed to go cold before eating (can be reheated, and remember cooked food goes off quickly so store it safely) or something like inulin (banana flour is rich in it) or acetic acid (vinegar) are easy ways to boost lower gut health, by providing inputs which favour the bacteria you want. You want a lower gut rich in buterate consumers.
If in 20 years time this cool, folksy self treatment is exploded, aside from being dead of old age, I won't care because i got to eat loads of potato lutkes, arancini, pasta reheats, yummy yoghurt and kefir and vinegary dressings on salad. Dying of old age eating nice food for no provable health upside? Fine with me!

Don't disagree with regards to the speed and efficacy of fecal transplants, they just don't scale is all.

The FDA approved oral FMT gelcaps in 2023. So, there's progress. That was to treat C. Diff.
