Context: This document grew out of repeated failures where autonomous systems made locally “correct” decisions that led to irreversible downstream effects (API calls, capital movement, infra changes). Existing guardrails, approvals, and monitoring didn’t help because the issue wasn’t decision quality—it was execution authority at the moment an action occurs. This write-up attempts to define a stricter execution-governance layer at that boundary, where suppression under uncertainty is treated as the correct outcome. Curious how others here are handling this.
